They happen when Operations have.

Weak supervision,
Normalized shortcuts.
That makes it easier to continue than to intervene.

"Safety is handling it."
Hazards get reported,
But never corrected at the source.
Procedures get followed.
Only when someone is watching.
Near misses become paperwork,
Instead of operational learning.
Safety becomes a department’s responsibility,
Instead of an operating discipline.
A stronger framework works like this.
Leaders set the expectations like.
Priorities,
Provide resources,
Keep people accountable.
Workers manage risk in the field.
Identify,
Communicate,
Control conditions.
Safety helps the organization.
See what it may be missing,
Then verify that the system is working.
You know if you have a good safety system.
When a supervisor can explain.
The risks,
The hazards,
The controls.
